Corticosteroid injections used to treat osteoarthritis pain in the hip and knee may be more dangerous than previously thought, according to a special report published in the journal Radiology. It found that it was more difficult to measure changes in bone in patients receiving cortisone-based injections and to compare these changes to healthy patients receiving a placebo, and, moreover, that these changes tended to be transient, although they appeared to be correlated with improved performance in movement. "As such, some clinicians who administer these injections may want to pause while they assess the safety and efficacy of their treatments," says lead author Paul W. Sperling of the University of California, San Francisco School of Medicine in the United States. A previous study found that using a low dose of corticosteroids may increase stroke risk. Because corticosteroid injections are effective in treating osteoarthritis pain for only short periods, they are often used off-label for treatment of other conditions. Previous findings have shown that low-dose corticosteroids increase bone mineral density and lead to an increased risk of hip fractures. "The new research suggests that, unlike a few years ago, in clinical settings, low-dose corticosteroids are just not as effective as clinicians used to believe," says Robert J. M. Cone, MD, director of the Division of Osteoarthritis of the National Institute of Arthritis and Musculoskeletal and Skin Diseases in Bethesda, Maryland. The results were surprising because researchers had estimated that using low doses of corticosteroids resulted in no increase in bone resorption. "I think this report is a real shame for patients," says lead author James N. Ladd, MD, a clinical professor of urology at Stanford University School of Medicine in California, "because there is no difference in the effectiveness of low doses of corticosteroids—both on normal tissue and when taking cortisone." There are several factors, including whether treatments are given at home or at a clinic, that can influence bone density, according to the authors, including factors like sex, height, bone density, and ethnicity. Because the researchers only looked at CT scans in patients with chronic osteoarthritis pain, they couldn't determine if such low-dose treatment would also increase fracture risk. To assess bone density, researchers used CT scans of a patient's femur and tibia before and after receiving low-dose corticosteroids and placebo injections. The researchers used scans that showed the density of each bone segment in combination, rather than as isolated images. This was done to avoid any differences in density found in individual segments.
